Participation Agreement

Participation Agreement: I, the adult applicant or I, the parent or legal guardian of the applicant listed above, hereby gives approval of the applicant’s participation in any and all programs and activities registered below. I do waive, release, absolve, indemnify and agree to hold harmless the organizers, sponsors, supervisors, participants and persons involved in the operation of programs for any claims arising out of injury or other loss to named applicant as participant or all present.

Health and Safety Agreement: I agree to wear a face-covering when inside the studio.I agree to follow all health and safety rules.

Photo Release: I also give permission for Akron ArtWorks to take photos of any of the participants listed above to use for promotional purposes or for any form of advertising.
